How to prepend digits to called id in MGCP

Based on the ISDN number plan and type of the incoming call, the following values will be used to prepend the Caller ID.
National number prefix
International number prefix
Subscriber number prefix
Unknown number prefix

CIR calculation

WHen calculating cir and mincir for FRTS, use 95 % of the CIR value. If CIR = 256k, it should be used as 256000. Do not multiply 256 with 1024.
